DRK personal ausweiss

I have a personal ausweiss for my wife, have reserched what goes where but have a question.

On the third page next to a space for a stamp, it says Ort und Datum, place and date; is that where the ausweiss was stamped or where DRK training took place?

On page 75 of 'papers please' it shows an original pass for Meta Hesse a DRK hilferin.
Page 3 is marked as follows:
name. vorname
Hesse Meta
DRK-Dienstgrad:
Helferin
Geburtstag-und ort:
17.10.1891 Ilfeld (ort is area/district)
Anschift (address)
Nordhausen,Bahnhofsplatz 3
Then finally were it states ort und datum :Nordhausen 10.10.40
rubber stamped DRK Kriestelle Nordhausen,(signed) DRK -Oberfeldfuhrer
Fur Bereitschaften (m) : 1.10.39
Deiser Ausweis gilt als Sanitatsschein Des DRK
All details have been typed except signiture and 1.10.39
